抗性糊精/桑叶提取物降血糖功效的系统评价与Meta分析

摘要:目的 探讨抗性糊精/桑叶提取物的降血糖功效。方法 系统检索数据库中抗性糊精、桑叶提取物与血糖管理相关临床研究,提取关键指标数据,进行Meta分析。结果 共纳入21项研究,其中抗性糊精8篇,桑叶提取物13篇。结果显示,抗性糊精干预后FBG[SMD=-0.67,95%CI(-0.86,-0.48),P<0.00001]及胰岛素抵抗指数(HOMA-IR)[MD=-3.05,95%CI(-3.36,-2.74),P<0.00001]均较治疗前下降。同时,抗性糊精干预后空腹血糖(FBG)[SMD=-0.91,95% CI(-1.73,-0.09),P=0.03]及糖化血红蛋白(HbA1c)[MD=-0.45,95% CI(-0.89,-0.01),P=0.04]水平均低于对照。与治疗前及对照相比,桑叶提取物干预后均显著降低 FBG[SMD=-1.12,95% CI(-1.81,-0.44),P=0.001;SMD=-0.68,95% CI(-1.21,-0.15),P=0.01]、餐后2h血糖(2hPG)[SMD=-1.59,95% CI(-3.03,-0.41),P=0.03;SMD=-1.01,95% CI(-1.52,-0.49),P=0.001]、及 HbA1c [MD=-0.64,95% CI(-1.04,-0.24),P=0.002;MD=-0.31,95% CI(-0.61,-0.01),P=0.04]水平。结论 现有研究证据表明,膳食中补充抗性糊精或桑叶提取物在改善血糖方面均有显著效果。

Abstract:Objective To investigate the hypoglycemic effect of resistance dextrin(RD)/mulberry leaf extract(MLE). Methods Systematically search the databases for clinical studies related to RD,MLE and blood glucose management,extract key indicator data,and conduct Meta-analysis.Result A total of 21 studies were included,among which 8 were related to RD and 13 were related to MLE. The analysis results showed that after the intervention with RD,FBG [SMD=-0.67,95% CI(-0.86,-0.48),P<0.00001],and [HOMA-IR(MD=-3.05,95%CI(-3.36,-2.74),P<0.00001]were lower than those before treatment. Meanwhile,after the intervention with RD,FBG [SMD=-0.91,95%CI(-1.73,-0.09),P=0.03],and HbA1c [MD=-0.45,95%CI(-0.89,-0.01),P=0.04] were lower than those of the control group. Compared with the pre-treatment and the control group,the intervention of MLE significantly reduced the levels of FBG [SMD = -1.12,95% CI(-1.81,-0.44),P = 0.001;SMD = -0.68,95% CI(-1.21,-0.15),P = 0.01],2hPG [SMD = -1.59,95% CI(-3.03,-0.41),P = 0.03;SMD = -1.01,95% CI(-1.52,-0.49),P = 0.001],and HbA1c [MD = -0.64,95% CI(-1.04,-0.24),P = 0.002;MD = -0.31,95% CI(-0.61,-0.01),P = 0.04]. Conclusion The current research evidence indicates that supplementing the diet with RD or MLE can both significantly improve blood sugar levels.
