西洋参抗疲劳功效的系统评价和Meta分析:动物模型和临床研究

摘要:目的 对西洋参抗疲劳作用进行评价。方法 计算机检索医学数据库,对动物及人类研究予以系统文献检索,对所选研究进行系统评价和Meta分析。结果 纳入26项研究,其中19项为动物研究,7项为人体研究。在动物模型研究中,对抗疲劳关键指标的Meta分析表明,与对照组相比,西洋参可提高耐缺氧能力[MD=7.86,95%CI (4.32,11.40),P<0.0001]、负重游泳时间[MD=5.47,95% CI (3.79,7.15),P<0.00001],并增加肝糖原合量[SMD=1.70,95%CI (1.38,2.03),P<0.00001],同时降低血尿素氮[SMD=-1.20,95% CI (-1.51,-0.89),P<0.00001]及乳酸合量(SMD=-1.53,95% C1 (-2.17,0.88),P<0.00001]。分析结果证实西洋参通过调节组织细胞代谢,增强耐受力,发挥抗疲劳作用。这在人体临床研究中得到补充,西洋参在慢性疲劳综合征、癌症相关疲劳及青少年运动性疲劳中均有明确的抗疲劳效应。结论 基于动物模型及人体临床研究的充分证据,西洋参具有抗疲劳功效。但仍需进一步开展针对西洋参的随机对照试验,以更科学的方式论证其应用潜力和确切疗效。

Abstract: Objective To evaluate the anti-fatigue effect of American Ginseng. Methods Computerized literature search was conducted in medicaldatabases. Systematic literature review and Meta-analysis were performed on the selected studies. Resuts A total of 26 studies were included, ofwhich 19 were animal studies and 7 were human studies. In the animal mode studies, the Meta-analysis of ky indicators for anti-fatigue showed thatcompared with the contol group, American Ginseng could improve hypoxtia tolerance [MD=7.86, 95% CI (4.32, 11.40), P<0.0001, swimmingtime under load [MD=5.47, 95% CI (3.79, 7.15), P<0.00001], and increase liver glycogen content [SMD=1.70, 95% CI (1.38, 2.03), P<0.000011, while reducing blood urea nitrogen [SMD=-1.20,95% CI(-1.51, -0.89), P <0.00001]and lactate content [SMD=-1.3, 95% CI(-2.17.0.88), P<0.00001]. The analysis results confired that American Ginseng exerts an ani-fatigue effect by regulating tissue cell metabolism andenhancing tolerance. This was supplemented by human clinical studies, where American Ginseng had clear anti-fatigue effects in chronic fatiguesyndrome, cancer-related fatigue, and adolescent exercise -induced fatigue. Conclusion Based on suficient evidence from animal models and humanclinical studies, American Ginseng has anti-fatigue cfficacy. Further randomized controlled trials targeting American Ginseng are still neded to morescientifically demonstrate its application potential and exact efficacy.
